Request Your Medical Records

Our Release of Information (ROI) staff will be happy to assist you with requests for your medical records. 

Release of Information Offices are located in the following locations:
            - La Jolla, Room 1589
            - Oceanside, Main Entrance
            - Mission Valley, 2nd Floor, Room 2000
            - Chula Vista, 2nd Floor, 1st window on the right

Office Hours: 8:00 a.m. - 4:00 p.m., Monday - Friday, excluding federal holidays

We can assist you with the following:

  • Access to review your medical records
  • Obtaining copies of your medical records
  • Requests to amend your medical records
  • Access to My HealtheVet
  • Requesting providers to complete forms or letters on your behalf

How to Request Information

To request your medical record for yourself, please complete VA Form 10-5345a - Individuals’ Request for a Copy of Their Own Health Information

To request records to be sent to a third party,  please complete VA Form 10-5345 - Request for and Authorization to Release Medical Records or Health Information

Complete the requested information, sign the form and either:

  • Mail to:
    VA San Diego Healthcare System
    Release of Information (ROI) Section, Health Information Management Service (HIMS) (136C3SV)
    3350 La Jolla Village Drive, San Diego, CA 92161
  • Fax to:  (858) 642-6414
  • Send a My HealtheVet secure message to *SD Release of Information (ROI) $ (requires a Premium MHV account), or
  • Bring in-person to the Medical Center or clinics (Mission Valley, Oceanside, Chula Vista, Escondido and Imperial Valley)

There is no cost to send copies directly to another health care provider. If copies are for a patient's personal use, photocopying fees may be assessed.
