If you are not enrolled for Appointment Lookup, the function will be available on My HealtheVet in Spring 2008.  *(If you are already enrolled with the VA San Diego Healthcare System for Appointment Lookup, that function will continue to be available until the national appointment look up program goes on line, sometime in Spring 2008).

To access your personal patient information (e.g. prescription refill and to view your medication names) you must register via the In-Person Authentication (IPA) process at the local VA medical center or clinic (see list of clinics below).

In-Person Authentication Instructions

1. Register at My HealtheVet (MHV): www.myhealth.va.gov
2. View the orientation video on-line or at one of the VA Clinics listed below.
3. Show one photo government ID such as military ID, driver’s license, or passport and show your VA Identification Card (VIC).
4. Fill out the MHV Participation VA Form (10-5345a) at one of the VA Clinics listed below.
5. Sign up for MHV and complete the In Person Authentication Process (IPA) at:
   • VA Medical Center La Jolla Patient Health Library 1st floor, east of elevators Room 1493D
   • VA Mission Valley Clinic Member Services
   • VA Vista Clinic Member Services
   • VA Chula Vista Clinic
