Clinical Trainee Check Out

For trainees completing a program and leaving VASDHS:

• Verify that you have completed/signed all progress notes, orders, and patient requests
• Turn in your VA ID badge to room 1593
• Check out with your Service by turning in your VA Meal Card, cleaning out your VA mailbox, returning any VA owned equipment (flash drive, etc.)
• If you haven't already, please complete the Learners' Perception Survey

If, for any reason, you are staying on at the VA past the end of the academic year please do not check out but contact the VA Education Service, ext. 1141
