What is the OEF/OIF Care Management Team?

This team serves to transition the health care of injured and ill combat service members and veterans from DOD to VA, while working to maximize each veteran's services and benefits.  The OEF/OIF Care Management Team is the central hub that provides comprehensive knowledge and care coordination specifically for recently returned combat veterans.

The OEF/OIF Care Management Team consists of a Program Manager, Case Managers, Transition Patient Advocates, and VA Liaisons for Healthcare.

What is an OEF/OIF Case Manager?

An OEF/OIF Case manager is a VA employee assigned to ease the transition process and serve as a liaison for the veteran in interactions with the VA system and community resources.   Case Managers are clinical team members.

What is a Transition Patient Advocate (TPA)?

A Transition Patient Advocate is your personal advocate who will assist you in obtaining the services and benefits you are entitled to.  The TPA provides information and assistance and acts as your advocate regarding benefits, entitlements and eligibility to health care and veteran benefits.  TPAs are administrative team members.

What is a VA Liaison for Healthcare?

VA Liaisons for Healthcare are assigned to the military treatment facilities.  Liaisons facilitate the transfer of care from DoD to VHA.  VA San Diego has 2 VA Liaisons for Healthcare assigned to Camp Pendleton and Balboa Naval Medical Center.

What is the OEF/OIF Program Manager?

The OEF/OIF Program Manager provides staff support and oversight for the effectiveness and quality of the OEF/OIF Care Management Team.
